The American Dental Association offers the DAT at Prometric testing centers in the United States and Canada. Hundreds of DAT testing centers are available.
Information about the locations of Prometric testing centers is available online atwww.prometric.com. Or you can call the Prometric Contact Center at (800) 688-5804.
Prometric will not seat candidates who arrive late for the DAT. We recommend that you schedule your travel time so that you reach your chosen DAT testing center at least 30 minutes prior to the beginning of the test. You must bring two forms of ID, and both forms must bear names that are identical to one another, as well as the name on your application.
In accordance with the Americans With Disabilities Act, the ADA and Prometric will make accommodations for covered disabilities. If you require accommodations, you must complete and submit a Testing Accommodation Request Form when you initially apply for the DAT, along with documentary proof. Once you have been approved for testing accommodations for the DAT, if you need to retest or to test at another Prometric testing center, it will not be necessary to request such accommodations again.
